Account Cancellation Form
If you wish to cancel your account with CommKal Hosting Solutions, you must submit a signed cancellation notice. We have provided a cancellation form for your convenience. Your cancellation notice must either be faxed to the number specified on the form, or it can be sent via postal mail to:

Managed Server Forms
To order a Managed Server account, each one of these managed server forms must be completed, signed, and faxed back to the number specified on the forms.
